Application Forms and Instructions
For any application, contact program staff for the most current information on submission. At this time, applications are submitted using either PHS Form 398 (paper) or SF 424 (R&R) (electronic). See section below for details on electronic submission.
You will need to know if a Letter of Intent is requested for the announcement to which you are responding. Please submit if requested.
Also, advance permission from NIMH is required for:
- Applications that are at or exceed $500,000 annually in direct costs; and
- Projects to support conferences and scientific meetings.
